I hereby give permission for my child to participate in Tab Ramos Soccer programs. I assume all risks in regard to the programs in which my child will participate. I release, indemnify and agree to hold harmless Tab Ramos Soccer Programs, it's directors, officers, coaches, trainers and volunteers from any liability that may arise from participation in the programs organized by Tab Ramos Soccer Programs. By agreeing to these terms, I attest to the following: that the information entered is correct, and in the event of a medical emergency, I authorize Tab Ramos Soccer staff to seek emergency medical care for my child as deemed necessary
